Your experience, goals and information for a safer start to coaching. Complete only what you wish to discuss with your coach.

This form sends nothing and stores nothing on a server or in browser storage. Information stays in the open page and is cleared when you leave or reload it. To retain it, print it or choose Save as PDF in the print dialog. Hand the completed form to the coach in person; do not put health information in the ordinary website enquiry.

3. Health history - optional section

The health section is optional. You need not provide diagnoses, full medical records or Vital Praha results. Instead of details, discuss relevant limitations or suitable medical confirmation with the coach. Without necessary fitness information, it may not be possible to recommend a particular exercise load safely.

Has a doctor restricted physical activity? Include only advice relevant to training.

Current pain, recurring injuries or operations affecting running

Past or current conditions that need to be considered during exercise

Treatment or medication affecting exercise - relevant information only, if you wish to share it

Most recent sports/medical examination and exercise advice, if available

Other information relevant to safe coaching

4. Declaration and separate consentI agree to participate in training with awareness of the ordinary risks of sport and personal responsibility for respecting my health and abilities. I will follow reasonable coaching instructions, report limitations and stop exercising if problems arise. This is informed acceptance of ordinary risks, not an exclusion or restriction of the coach’s statutory liability for harm.

Separate consent for health information

I expressly and voluntarily consent to Mgr. Marek Oliva, business ID 74498100, U Hadovky 598/26, 160 00 Prague 6, processing only the health information I provide to adapt and safely deliver coaching under Articles 6(1)(a) and 9(2)(a) GDPR. This consent does not cover marketing or publication.

The coach has access to the health questionnaire handed over. Data is retained during coaching and for no longer than 6 months afterwards, or until consent is withdrawn, then erased or destroyed. The only exception is information necessary for a specific legal claim under Article 9(2)(f) GDPR. Health information is not automatically shared with Vital Praha or used for advertising.

Consent may be withdrawn at any time at marekoliva@gmail.com or by writing to the controller’s address, without affecting earlier lawful processing. You may request access, correction, erasure, restriction and portability where the GDPR conditions apply; complaints may be made to ÚOOÚ (uoou.gov.cz). Ordinary contact and training information is processed to perform the agreed service, not under this consent.
